FUE serves as an alternative other than strip in donor harvesting (1). It attracts attention of those who desire hair transplantation but are turned off by a linear donor scar.<\/p><p>Being a blind technique, the number of grafts is limited by a high transection rate. Over the years many different FUE techniques have evolved to overcome this problem (2)(3)(4). A 5-steps personal approach will be described in part one of this chapter. \u00a0<\/p><p>The real problem of FUE however is the depletion of donor grafts for future sessions. Tissue debris can be irrigated out through the slot of the handle.<\/li><li>Depth control of the punch can be set<\/li><li>The weight and length of the handle helps to control the tip on scoring.<\/li><\/ol><p><strong>C<\/strong><strong>urved forceps<\/strong> provides a larger contact area for a better grip of graft. A second pair of <strong>straight forceps<\/strong> is needed for difficult extraction.<\/p><p><strong>S<\/strong><strong>urgical loupes<\/strong> provides good visualization.<\/p><p>Magnification higher than 2X may cause more<\/p><p>eye-strain.<\/p><p><strong>M<\/strong><strong>oist dressing material<\/strong> instead of gloved finger should be used to collect extracted grafts <strong>(Fig 4) <\/strong>before transferring to a <strong>Petri-dish with cool saline<\/strong> (7).<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-647e8ef elementor-widget elementor-widget-heading\" data-id=\"647e8ef\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">1.2. Preoperative preparation<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e1c1e7c elementor-widget elementor-widget-text-editor\" data-id=\"e1c1e7c\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Grey or white hair should be colored before surgery. FOX test has been advocated for patient selection especially in megasession (8). In real practice it is rather time-consuming and unreliable. What initially appears to be difficult may turn out to be easy extraction after the surgeon adapts the technique to cater for the angle and direction of the follicles. The entire donor site should be trimmed leaving behind 0.3-0.5cm of hair above the skin to guide the punch. Those reluctant to shave must have kept hair long enough to cover one or two trimmed windows.<\/p><p>Scalp laxity should be assessed (9). Very tight scalp alerts the possibility of underlying idiopathic tissue fibrosis which necessitates deeper scoring.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-f0328ad elementor-widget elementor-widget-heading\" data-id=\"f0328ad\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">1.3. Positions<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-78679ab elementor-widget elementor-widget-text-editor\" data-id=\"78679ab\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Patient\u2019s comfort must be ensured as FUE takes many hours. The patient should lie in a prone position with the surgeon sitting either by the side or at the end of the table.<\/p><p>Inspect the hair at the occiput and parietal areas. The surgeon should start at an easy area to gain confidence.<\/p><p>The patient\u2019s neck should be flexed when harvesting the inferior donor area. Here the hair usually exits at flat angle and stretching the scalp helps to erect the follicles.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-9bb756c elementor-widget elementor-widget-heading\" data-id=\"9bb756c\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">1.4. Marking the FUE donor area<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-836d883 elementor-widget elementor-widget-text-editor\" data-id=\"836d883\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>The \u2018safe margins\u2019 of the donor area should be marked to avoid picking up non-permanent hair. There is no fixed rule in defining the safe donor areas. However Unger and Alt\u2019s recommendations may be used as guideline (10).<\/p><p>Looking for grafts at the end of procedure is frustrating. One practical tip is to divide the marked donor area into small blocks of similar sizes.<\/p><p><strong>N<\/strong> = The planned number of grafts<\/p><p><strong>n\u00a0<\/strong> = The number of blocks<\/p><p><strong>N \/ n<\/strong> = The number of grafts to be harvested per block.<\/p><p>If this number cannot be obtained in a defined block, the surgeon should prepare to expand the donor area or accept lesser grafts.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-7b23154 elementor-widget elementor-widget-heading\" data-id=\"7b23154\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">1.5. Local anaesthesia &amp; tumescence<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-81c7d9e elementor-widget elementor-widget-text-editor\" data-id=\"81c7d9e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>L.A. of personal choice should be injected below and along the inferior border.\u00a0\u00a0 Tumescence should not be injected as routine. The ease of extraction must first be tested with and without tumescence. It should be injected intra-dermally a small area at a time, and away from \u2018holes\u2019 to avoid spilling.<\/p><p><strong>Graft selection <\/strong><\/p><ul><li>From personal experience a random approach may obtains more multi-FU grafts than an orderly approach in extraction.<\/li><li>All healthy looking multi-FUs within a block should first be randomly extracted. On the second round 1-hair grafts are extracted.<\/li><li>One rule must be followed: those grafts next to an empty hole should be spared.<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-3b8631c elementor-widget elementor-widget-heading\" data-id=\"3b8631c\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Part 2 : The 5-Steps approach<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-65b8ea0 elementor-widget elementor-widget-text-editor\" data-id=\"65b8ea0\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Straighforward FUE requires only 2 steps \u2013 scoring and extraction (11). When extraction is difficult with high transection, a 5-steps approach is necessary.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-92b290c elementor-widget elementor-widget-heading\" data-id=\"92b290c\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Step 1 : Identation<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-c7d8488 elementor-widget elementor-widget-text-editor\" data-id=\"c7d8488\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<ul><li>The sharp tip of the punch may slip and transect the underside of the follicle. It should be secured on the skin surface before scoring.<\/li><li>After containing the hairs of a FU, the punch is positioned vertically on the skin surface.<\/li><li>It is then pushed downward to create an indentation on the skin, which in return embraces the punch <strong>( Fig 1 ).<\/strong><\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-3bf060a elementor-widget elementor-widget-heading\" data-id=\"3bf060a\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Step 2 : Sharp scoring<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-888bb81 elementor-widget elementor-widget-text-editor\" data-id=\"888bb81\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<ul><li>The angle of the punch is changed to align with the hair.<\/li><li>The handle is rotated to-and-fro with the thumb, index, and middle fingers to advance the punch.<\/li><li>Scoring should stop when tissue resistance is felt. This usually occurs on cutting the Arector pili at the isthmus (12).<\/li><li>Beyond this point the tissue will suddenly give way so that the punch may drop too deep and transect the roots <strong>( Fig 2 )<\/strong><strong>.<\/strong><\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-4d47ed2 elementor-widget elementor-widget-heading\" data-id=\"4d47ed2\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Step 3 : Test extraction<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-094d539 elementor-widget elementor-widget-text-editor\" data-id=\"094d539\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<h5>Single forceps technique &#8211;<\/h5><ul><li>One common problem encountered in extraction is \u201cde-capping\u201d \u2013 by just grasping a small amount of tissue the skin only is removed without the follicle.<\/li><li>The tips of the curved forceps are opened, placed around the dissected follicle, and passed down upon the surrounding skin.<\/li><li>The exposed upper dermis of the popped up follicle is then grasped firmly by the same forceps.<\/li><li>The graft is \u2018pushed\u2019 rather than \u2018pulled\u2019 out by pronating the wrist <strong>( Fig 3 )<\/strong>.<\/li><\/ul><h5>Double forceps technique &#8211;<\/h5><ul><li>If de-capping still occurs, the follicle is first held and lifted by the curve forceps as described.<\/li><li>A second pair of straight forceps is positioned below the curve forceps to grasp lower and extract the graft.<\/li><li>This step can be performed by an assistant.<\/li><li>Any de-capped graft should be left behind. Struggle to remove may create a buried graft.<\/li><li>There is a learning curve for each case. The first 15 to 30 minutes should be spent patiently to test the proper angle, direction, and depth of the punch.<\/li><li>Successful extraction requires no further action but if the graft fails to come out, the next two steps becomes necessary<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-ff4bf7a elementor-widget elementor-widget-heading\" data-id=\"ff4bf7a\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Steps 4 and 5 : Deep dissection and re-extraction<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-be93a77 elementor-widget elementor-widget-text-editor\" data-id=\"be93a77\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Three different tools can be used to free the follicle from any deep tethering before re-extraction.<\/p><ol><li>Using Dr Harris\u2019 dull punch (4)(13);<\/li><li>Using a sharp needle tip as recommended by Dr. Rassman (11);<\/li><li>Using the same punch to cut deeper but with a lesser force and speed.<br \/><br \/><\/li><\/ol><p>These two steps are repeated until the graft is removed.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-f18549b elementor-widget elementor-widget-heading\" data-id=\"f18549b\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Part 3 : FUE mega-session : special considerations in the Chinese populations<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-56e5eca elementor-widget elementor-widget-text-editor\" data-id=\"56e5eca\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>For these population the difficulties in FUE usually arises from a discrepancy between high demand (14)(15)(16)(17) and low supply of donor hair (5)(6). There are two ways to increase the number of grafts but at a higher risk for complications:<\/p><ol><li>By expanding the harvesting area into unsafe zones;<\/li><li>By extracting more hair per sq.cm.<\/li><\/ol>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-3e92f8e elementor-widget elementor-widget-heading\" data-id=\"3e92f8e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">3.1 Complications<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-11ae385 elementor-widget elementor-widget-text-editor\" data-id=\"11ae385\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<ol><li>Non-permanent hair is picked up by harvesting non-safe area.<\/li><li>FUE prevents a linear scar but it is not a scarless technique (1). As a larger area is utilized the cumulative scarring of open wounds is in fact significantly greater than from a linear incision.<\/li><li>Empty spots, snail-tracks, and mottling occur when adjacent follicles are extracted <strong>(Fig 5, 6)<\/strong>. These are more visible in Asians with high hair-scalp color contrast.<\/li><li>Thinning occurs due to the selective extraction of 2-3 hair FUs in a low density donor area (18). Any underlying scars or mottling becomes more obvious.<\/li><li>Depletion of donor density leading to a wider scar if strip is required for subsequent sessions.<\/li><\/ol>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e48abd4 elementor-widget elementor-widget-heading\" data-id=\"e48abd4\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">3.2 Advantages of FUE<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-2251482 elementor-widget elementor-widget-text-editor\" data-id=\"2251482\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<ol><li>One doctor and 1-2 assistants can run a center. 2.
