What Is Telogen Effluvium?

Telogen effluvium ( TE ) is one of the most common — and most reversible — forms of hair loss. It happens when a significant stressor pushes large numbers of growing hairs into the resting (telogen) phase all at once. A few months later, those hairs shed together, leading to noticeable thinning.

Unlike genetic balding, TE doesn’t destroy hair follicles. The roots stay healthy and ready to regrow — once the trigger is gone.

Common Triggers : What “Shocks” the Hair Cycle?

Telogen effluvium can be triggered by various physiological stressors that disrupt the normal hair growth cycle.

1. Postpartum Effluvium

Approximately 30-50% of women experience significant hair shedding 2-4 months after childbirth. This occurs due to hormonal changes as estrogen levels drop, causing hairs that were maintained in the growing phase during pregnancy to simultaneously enter the resting phase.

2. Significant Weight Loss

Rapid weight loss, particularly when associated with crash dieting or bariatric surgery, can trigger telogen effluvium. Nutritional deficiencies, especially in protein, iron, and zinc, contribute to this response.

3. Physiological and Emotional Stress

Major surgery, severe infections, high fever, traumatic accidents, or significant emotional stress can precipitate hair shedding. The body diverts resources away from hair growth during periods of high stress.

Acute vs. Chronic Telogen Effluvium

Acute Telogen Effluvium

  • Duration : Lasts less than 6 months
  • Onset : Sudden, noticeable shedding
  • Trigger : Usually identifiable (surgery, illness, medication)
  • Recovery : Complete regrowth typically occurs within 6-12 months
  • Management : Remove trigger, supportive care

Chronic Telogen Effluvium

  • Duration : Persists for more than 6 months
  • Onset : May be insidious
  • Trigger : Often multifactorial or unidentifiable
  • Pattern : Fluctuating course with periods of exacerbation
  • Management : Requires comprehensive evaluation for underlying causes

Is This Permanent?

No — true telogen effluvium is not permanent.
The follicles are still alive. Once the stressor is removed, new hairs begin growing — though it may take 3–6 months to see visible improvement, and up to a year for full thickness to return.

⚠️ Exception : If the trigger persists ( e.g., untreated iron deficiency or uncontrolled thyroid disease ), shedding continues. That’s why identifying the root cause matters.

Diagnosis : Ruling Out Other Conditions

Because TE mimics other types of hair loss (like early female pattern balding or diffuse alopecia areata), a careful evaluation is essential.

Your doctor may:

  • Take a detailed history (recent events, medications, diet, stress)
  • Perform a hair pull test (gently tugging hairs—if >6 come out easily, it suggests active shedding)
  • Order blood tests: ferritin (iron stores), TSH (thyroid), vitamin D, zinc
  • In unclear cases, a scalp biopsy or hair wash test may be used

🔍 Red flag: If thinning is only in the front or part widens significantly, it might not be pure TE—it could be androgenetic alopecia unmasked by stress.

Treatment: Patience + Supportive Care

There’s no “quick fix”—but you can support recovery:

✅ Do:

  • Address the trigger (treat deficiencies, manage stress, adjust meds if possible)
  • Eat enough protein, iron, and key nutrients
  • Use gentle hair care: avoid tight styles, heat tools, harsh chemicals
  • Give it time—regrowth is slow but steady

❌ Don’t:

  • Start minoxidil expecting miracles (it won’t speed up TE recovery)
  • Panic or over-wash/over-brush trying to “test” shedding
  • Rush into cosmetic solutions before stability is confirmed


Most importantly: reassurance. Knowing this is temporary reduces anxiety—which itself can worsen shedding.

Can You Get a Hair Transplant for Telogen Effluvium?

1. Absolutely not during active shedding.

Hair transplantation is designed for permanent hair loss — not temporary disruption. Operating during TE risks:

  • Wasting precious donor hair
  • Poor graft survival
  • Misdiagnosing underlying genetic balding

2. When might surgery be considered?

Only after:

  • The trigger has been fully resolved
  • Shedding has stopped for at least 6–12 months
  • New hair is clearly regrowing and stable
  • A specialist confirms no underlying androgenetic alopecia was unmasked

🚫 Never transplant into a scalp that’s still shedding. Wait. Watch. Confirm stability first.
